Topic: Workers’ Compensation (Claim Form)

Arizona now requires Employer Report of Injury (Form 101) to be submitted electronically via their WebForm system. 

The ICA has issued a has launched modernization of the Employer’s Report of Injury Form (101). This initiative was undertaken by the Claims Division, MIS teams, and ICA’s developers developed in conjunction with stakeholders. Under current law, ARS§ 23-908(G) and
23-1061(E) require the Employer to notify the carrier & the Commission within 10 days on the form and in the manner as prescribed by the commission. Specifically, the Employer’s Report of Injury Form has been modernized as follows:

  • Streamlined by almost 50% fewer questions.
  • The form revision has eliminated duplicate questions that are required on other occupational injury forms and also simplified the information required of the employer.
  • Available only electronically as a webform on the ICA Claims Employers website.
  • Effective August 30, 2025, existing versions of Form 101 will no longer be authorized for use, and all previously authorized alterations of the forms will expire.

Customers should continue to report claims to CorVel.
